There are a couple of places that you may need to visit to
report your Health Savings Account (HSA) information in your return if there were either contributions or distributions made during 2017.
First, if any contributions are made through payroll deductions, the W-2 will show an amount in Box 12 with Code W.
Second, if you used some of the money in the HSA during 2017 you should receive a Form 1099-SA that will report the distributions. This form will need to be entered in the return.
Finally, there is a section dealing with HSAs that can be found in the Deductions and Credits section of your return. The information entered there will verify that you were eligible to contribute to the HSA based on your coverage by a High Deductible Health Plan (HDHP).
