Hello,
I am having an issue, I worked from home for the entire year, however my employer continued to take out taxes to the municipality I used to work in. I live in a RITA tax area, so I also paid my tax bill to the municipality that I actually live in.
Essentially, I ended up paying double taxes when I didn't need to. How can I go about getting this corrected and/or getting a refund from the city that I had taxes deducted from that should not have been paid? How do I prove I worked from home for the entire year as well?

You will have to file a 2nd City return, for the city your employer withheld for, to get a refund. Ask your employer to stop withholding for that city. You should contact that city, as many have a special form for a refund in this situation.
During Covid, Ohio allowed cities to collect tax, from the work city, even when the employee worked from home in a different city. That ruled expired 12-31-21. Your employer may not be aware of that expiration.

On the website they ask for "official clock hour summaries (telework/regular hours,
benefit leave times)"
What kind of documentation would be needed to support this? My company switched timesheet software through the middle of this year so I don't have access to half a year of these.
You'll have to ask the city what they required. The one time I had to do it, I had to get my supervisor to sign a city affidavit form.
