Missing form 8889 for HSA Adjustment

I have employer contribution to HSA. If I understanding it correctly, HSA contribution is nontaxable in federal tax, and I am required to submit 8889 to adjust it. However, this form is not generated by Turbotax. How can I get it include form 8889?

Missing form 8889 for HSA Adjustment

If your employer made the contribution you are not entitled to a deduction  on your federal return.  If you also contributed to this HSA through payroll deductions, those deductions were done pretax, therefore also not deductible on your federal.  

 

TurboTax will generate a Form 8889 for you, however this only applies if you made an HSA contributions in additional to the payroll contributions.  This additional contribution would be deductible on your federal return.

follow up from earlier post....the employer contribution (theirs plus your payroll contributions) should be stated in box 12 on your W2.  When you enter that box 12 value into TurboTax, TurboTax should generate a form 8889.  That value will not be deductible, but will count towards your annual maximum ($7000 + $1000 if over 55)
