If this was the sale of your principal residence, and you qualify to exclude the gain on the sale, and you didn't receive a 1099-S, you don't have to report it.
See IRS Publication 523, page 18.
"Determine whether you need to report the gain from your home. You need to report the gain if ANY of the following is true:
If NONE of the three bullets above is true, you don’t need to report your home sale on your tax return."
If you do have a gain to report, and you don't have a 1099-S, report your sale with the information from your closing statement.
