If your spouse has a code W in box 12 on his/her W-2, then your spouse has an HSA, which has to be reported on form 8889 as part of your return.

The form is 8889 - the 8889-T is for the primary taxpayer and the 8889-S is for the spouse.

Does your spouse have an HSA? Is there a code W in box 12 of your spouse's W-2? Did your spouse receive a 1099-SA? If there was an HSA and there was any activity in the HSA in the current tax year, then the form 8889 must be added to the return.

If your spouse is not aware of having an HSA (despite the code W in box 12 of the W-2), then your spouse needs to contact his/her HR department and find out the benefits of having an HSA.

If neither of you has an HSA, then you need to remove the data that is causing this situation:

1. make a copy of your W-2(s) (if you don't have the paper copies)

2. delete your W-2(s) (use the garbage can icon next to the W-2(s) on the Income screen

*** Desktop***

3. go to View (at the top), choose Forms, and select the desired form. Note the Delete Form button at the bottom of the screen.

*** Online ***

3. go to Tax Tools (on the left), and navigate to Tools->Delete a form

4. delete form(s) 1099-SA (if one), 8889-T, and 8889-S (if one)

5. go back and re-add your W-2(s), preferably adding them manually

Continue with your return.
