I didn't receive a 1099-SA for my medical payments from my HSA. What do I do?

If you had distributions from your HSA account during 2018, you should have received a Form 1099-SA by the beginning of February.  I suggest contacting the Financial Institution that houses your HSA to see if you can access the form online or have the form mailed to you.
