I didn't get a form 1099-SA. What do I do?

Form 1099-SA - You would receive this form if you used HSA funds to pay for qualified medical expenses. It reports disbursements from the fund.

If you did not have any disbursements, then you will not receive the form. If you did have disbursements, then contact the custodian of the fund for the form.
