I did not get a 1099 HC Form so what should I enter for name, and subscriber number, and federal identification number?

Don't you have a subscriber card? Is it possible that you can get this information from the website of the insurance company?

I do but it only has my own member id and not much else, their website has  no  way to retrieve the information  that ive  seen and  theres no  option  on turbotax to say i havent received  a 1099-hc form so im at a loss

Then I guess you can file an extension, and work on this problem next week.

Call whatever insurance company you have and ask them for a copy of the 1099-HC.
