FSA contributions are reported on your spouse's W-2 and are a pre-tax deduction from their income.
There is no need to separately report an FSA contribution.
If your wife's FSA can be used to cover your general medical expenses, it is coverage that disqualifies you from being eligible to contribute to an HSA. (Revenue Ruling 2004-45)
