I checked the incorrect box for the months OCT, NOV & DEC on the smart form 8889-T, how can i change it to the correct box?

I take it that you have tried just changing the boxes with no success.

If so, one sure way that will work is this:

• Delete your W-2(s) (but make sure you have a copy of it(them))
• Go to Tax Tools on the left, then navigate to Tools->Delete a form.
• Delete the forms 8889-T, 8889-S (if one), and 1099-SA (if one)
• Re-add your W-2, but enter it manually (not automatically imported)
• Return and complete your HSA interview
• Proceed with the rest of the tax return.

This will remove all the HSA data to let you start over.
