Are you a W-2 employee?
Sorry---W-2 employees cannot deduct job-related expenses on a federal return. Job-related expenses were eliminated as a federal deduction for W-2 employees by the tax laws that changed for 2018 and beyond. Your state tax laws might be different in AL, AR, CA, HI, MN, NY or PA. Commuting to and from work is never deductible--even late at night. Sorry.
If you receive a W2, you can not deduct for mileage. That deduction was removed for years 2018-2025.
If you are self employed or a 1099 employee:
If the hospital is your regular worksite, then that is considered a normal commute.
Commuting to a regular worksite is not considered compensable or deductible. If they were having you go to another location other than your regular worksite, that would be compensable or deductible.
It depends. If If you are a W-2 employee, under the Tax Cuts and Jobs Act (TCJA), the deduction for these 2% miscellaneous expenses has been suspended in tax years 2018 through 2025.
