How do I find Form 5498-SA on the app?

You won't find it on the app.  That form is sent to you for informational purposes only and doesn't need to be entered into the program.  If you have a Health Savings Account(HSA), make sure you go through the HSA interview under Less Common Income.

Form 5498-SA is not a form that will be attached to your tax return. It is used for tracking contributions to Health Savings or Medical Savings Accounts (HSA or MSA). It is for informational purposes only and is prepared by the custodian of the account. It is not accessible in TurboTax.
