Line 18 is on form 8889 in the section referring to “Failure to maintain HDHP coverage”.  TurboTax is asking for information from your 8889 on your 2016 return.

You triggered this when

  • you indicated that you were in the HSA on December 1, 2016 (yes, 2016) (see screenshot #1, “What type of high deductible plan did [name] have on December 1, 2016?"), and
  • you answered "Family" or "Self", and
  • you did not indicate in the HSA interview that you were covered by an HDHP in every month in 2017.

There are two major causes of this situation:

1. you did not complete the HSA interview (Federal->Deductions & Credits->Medical->HSA MSA Contributions), or

2. you don't have and never had an HSA, but answered the 2016 question with "Family" or "Self", not realizing that you didn't have an HDHP and that the questions didn't apply to you and that you should have answered "None".

In case #1, go back and complete the HSA interview, until you are returned to the "Deductions & Credits" menu screen.

In case #2, delete your HSA information and continue your return. Do the following:

1. make a copy of your W-2(s) (if you don't have the paper copies)

2. delete your W-2(s) (use the garbage can icon next to the W-2(s) on the Income screen

3. go to Tax Tools (on the left), and navigate to Tools->Delete a form

4. delete form(s) 8889-T, 8889-S (if one), and 1099-SA (if one)

5. go back and re-add your W-2(s), preferably adding them manually

Continue with your return.
