The provider on your Form 2441 is your daycare provider. As @SweetieJean said, it could be a person or a daycare.
In either case, to claim the credit, you will need their Social Security number (person) or Employer Identification Number (organization) unless the organization is a tax-exempt organization or foreign provider.
If you don't have the number, try your best to get it from your care provider and enter it on your return. If you can't get the number, you may not be able to receive the credit, and you will also not be able to e-file. (You can still print and mail the return without the EIN or SSN as long as you tried your best to get the number).
