Filling 1099-R for CaNADIAN RETIREMENT INCOME (RMD)

Hi,

I am having problem to fill my 1099-R.

I am a us citizen.  I get RMD from my US IRA account and RMD from CIBC, Canadian financial inst.

I get Canadian NR4 slip showing that CIBC has withheld my 15 % taxes.  When fill 1099-R for CIBC  the form ask me to fill foreign payer  Fedral ID which I do not have.  If leave Payer id blank , the RMD amount  and taxes will not show on my 1040.

Question is: How to report on 1099-R Canadian RMD income and taxes.

Thank you in advance.
