Child & Dependent Care (3506) form keeps complaining about missing address, etc,. about provider. All the info has been entered, yet it complains. How to solve it?

Hi there-

The 3506 is the California version of the Child and Dependent Care Credit. All of the information should have transferred from the Federal return into the CA return. There shouldn't have been much (if any) adjustment to make in California. I would go back first to the Federal and walk through the Child and Dependent Care flow again to make sure everything is entered properly. Then go back to California and walk through all of those screens again. Let us know if you are still having issues.
