I received a refund from a charitable contribution I made 5 years ago. Do I need to update any federal or state tax returns?

Yes, if you donated to a Charitable Contribution and were able to see a benefit from applying it as a deduction to a tax year, and then this Charitable Contribution was (for whatever reason) refunded to you, you would need to amend your return for that year IF and only IF the amount of benefit you received reduced your taxable income enough to put you into a lower tax bracket, thus reducing your "Tax" for that year.
