It asks Did you put some of your HSA contributions into wifes HSA. My employer only deducts from my account, I have my family on mine. She uses it.

An HSA account has only one owner, even though distributions from the account can be used to cover the medical expenses of others.  If there is only one HSA account and it is in your name, none of the contributions went into your wife's account because she has no HSA account.
