What form do I need to fill out for a Kentucky state return for an LLC that dod not do any business in Kentucky that year?

It originated in Indiana, and in 2015 we did business in Ky therefore added it as a "Foreign LLC" for Kentucky, and it is still active as such.

You file Form 725.  You will need to do so even though you had no activity.  If you no longer will be doing business in Kentucky, you will want to check the box that this is a Final Return (so you don't have to file and pay the $175 Limited Liability Entity Tax).  Also, make sure you verify with the Secretary of State to remove the LLC designation in KY.
