I have a question regarding the "Taxes paid to another state" section:
I am a student living in AZ and completed a summer internship in NY. Does that qualify for "taxes paid to another state" and does the "Total income tax" include both the "federal income tax withheld" and the "state income tax"?

Yes, If you had an internship in NY you should have gotten a W-2 to report it. I would expect that NY taxes would have been taken out. You will have to prepare both a NY and AZ tax return. Hopefully, the taxes withheld in NY will cover the income.
You will get a credit for the NY taxes on your AZ tax return as taxes paid to another state. Please note that AZ will only cover up to the amount the AZ tax would have been. You will not get a refund for additional taxes in NY.
