Third party authorization form

I signed the 3rd party authorization form and accidentally declined. I want to accept so I can apply for the loan but I can’t find the form anymore so I can change my answer. Where is the form?

Are you trying to apply for the loan advance?

If you haven't found the forms, please follow the instructions below:

• From your Tax Home menu click on the last tab Review and File, then click on Finish and file
• You will see three steps. The loan advance option will be under Step 2 your refund info, click continue
• Navigate through the following screen until you see How would you like to receive your federal refund?
• After you have selected the Refund Advance option you will come to the screens to view and sign the forms pertaining to the loan
