Additionally, for line 25, when an exception is met for that installment period, can someone confirm that 25-28 remain blank? I am working with exception #3 - Tax on Annualized Income.

Yes, lines 25-28 are blank for the periods you don't have an underpayment. Lines 14-19are blank when you're using an exception, so nothing should be on line 15.
