To the group, I just got off of the phone with NYS529 central and they do not have any knowledge of this requirement by NYS Taxation.
More importantly, they only keep records availalbe on line going back 7 years, so the request is impossible to do unless you have been tracking your contributions and withdrawals since inception.
This appears to be an Intuit data request. And if you do not answer it, the State will complete without the information, so my suggestion is skip over this and finish your taxes.

@keithmcphail2 it is not required to file but, helpful to maintain as in the future if you take the funds out to know your basis for potential taxes. TT is asking just like asking about IRA prior contributions so you maintain tax history all in one place.
