The payment voucher is Oklahoma 511 V. If you indicate that you wish to send in a check to the state of Oklahoma for your balance due, form 511 V will automatically be added as a list of forms in your return.
There must be an error in the printing. All forms will print EXCEPT the Oklahoma voucher forms. Please help!
As a workaround, you can open the form from the Oklahoma website, fill it in online, then print and mail it. The form is very simple.
The only number that you need from your return (the rest is personal information about yourself) is the Balance Due. You get this from form 511, line 43.
