Another possibility since we don't know what all you've tried.  If the employer included the letters "UT" along with the employer ID number, do not include the UT in the same number field.   The UT goes in a separate little sub-box.  Box 15 has 2 entries; the UT goes only in the first sub-box.   Some people have tried to include the UT in the same sub-box as the number.   And you've likely tried using lower case "wth" at the end of the number, but be sure you have done so on all W-2's and 1099's that have a state employer withholding tax number.
