The 1099-R shows a state I have not lived in for 4 years.
No taxes were taken and the taxable amount shown is below the filing threshold for the state.
I have contacted the bank (Chase BTW) and they refuse to correct it.
Should I:
1. Input the 1099-R into TurboTax as is with the wrong state.
2. Input the 1099-R with the correct state and if so, do I need to do any other documentation.
3. Chose to submit a substitute 1099-R and then also do the additional form 4852.

If there were no taxes withheld, that state code won't matter. You can enter the form as it is, TurboTax won't generate a tax form for that state and only use your resident state for any calculations. That is by default, so you don't need to make any changes to the form. Had there been state tax withheld for that state, you would have to file to get them back, but since no tax was withheld, no forms will generate.
Make sure they correct your address so it doesn't happen in the future.
