I have lived in Arizona now for three years. The Teachers' Retirement system, State of Oklahoma has withheld state tax of $120 but there in no entry in box 16. I am retired and living in AZ. Do I have to file a return for Oklahoma? How do I enter this information. Do I call OK and ask them not to withhold state tax? What is the easiest solution? Thank you for any assistance on this matter.

To file your Federal return, you will need to put the amount shown in box 1 of your Form 1099-R into box 16 if there is a state tax withholding reported in box 14. Otherwise you are probably seeing an error condition.
As for the withholding that was sent to Oklahoma, the only way to get it back would be to file a non-resident tax return for Oklahoma. You are not required to file a return for Oklahoma. It would be your choice.
Then, contact your retirement plan administrator and ask that they stop withholding Oklahoma state tax so that you will not have to file a tax return to get it returned to you.
