Turbotax shows that we owe Maryland State tax with penalty of 287 on the underpayment of estimated tax.

There is an option to pay the Maryland State tax without the penalty by letting Maryland bill me/us.

If we take that option of 'Let Maryland bill me', will there be additional interest on the penalty.

Yes, they will calculate for a future date and include the amount to pay by that due date. If you can't afford to make the full payment, pay what you can to reduce the balance and payments. You can make payments online anytime.
