Only last name presents in upper box in CA 540 form

Hi 

I printed my CA state tax return 540 and found out that only the last name present in the upper box. I will need to submit state tax return by mail. will it be an issue?

Your last name and social is all that is necessary. You can add your first name with a pen if you want to; but, it isn't necessary.
