Massachusetts - 1099-HC (Box 5 or Box 6 for Subscriber Number)

Turbo Tax says to enter subscriber number from my 1099-HC (Box 5).

However, Box 5 is DOB. Box 6 is my subscriber number.

Is Turbox Tax correct, or is my 1099-HC incorrect.
