And if so, what part is taxable- the fair market value of the book?

No, the book is not subject to your state sales tax.

Only purchases are subject to sales tax - you didn't purchase the book. You made a charitable contribution and the donee sent you a gift to show their appreciation for the donation.

I received a 1042-S in the mail. It is "Foreign Persons US Source Income Subject to Withholding". I am not sure what I am supposed to do with it, or if amend my taxes.
