I need to file a form 804 with my Wisconsin State Return. I have saved the PDF file in Documents, but I'm not sure how to attach it to the Wisconsin return?

That form is not supported by TurboTax. You will need to mail it in. You want to use tracking, no signature required. I am sorry for your loss.
