I live in Rockland, NY but work in NYC Do I pay NYC taxes or only New York State taxes or both, and do I have to fill out the 1127 form?

You pay state and a NYC nonresident tax. You pay a 1127 if you are city employee.
From the New York State tax website:
"All city residents’ income, no matter where it is earned, is subject to New York City personal income tax. Nonresidents of New York City are not liable for New York City personal income tax."
