I am looking for AZ Form 352 (Credit for Contributions to Qualifying Charitable Organizations)

In the Arizona state program, you should see a screen titled "Other Nonrefundable Credits"  (this is toward the end of the state interview screens).  See attached screenshot. 

To get to where you complete Form 352 just check the box on that screen and continue.
