If you note 125P and 414H on your federal return, do you need to note it a second time when it says to Enter Insurance and Benefits Surcharge on the state return?

Please clarify. What state are you referring to? What and where are you noting 125P and 414H?
Thanks for replying, Helen. This is for NY state. I've noted 125P and 414H on section 14 of my W2. I input this information for my federal tax return. When I fill out the state tax return, TurboTax says it transfers the info from my W2 to the state return (from the federal one). There is a section called "Changes to Federal Income," and then a line for "NYC health insurance or welfare fund withholding." If I mark the $ amount for 125P here, am I marking this amount of withheld money twice?
Yes, you are correct if you put in an amount in the topic line "NYC health insurance surcharge" after identifying the amount in box 14, it will be added to your NY income twice. You would only need to enter an amount in the "NYC health insurance surcharge" topic if there was a separate NYC retirement system item not reported in box 14 on your W2.
