Your issue is with the payer.  The IRS does not care if you got the money or not - only that you report the income.   The IRS receives a copy of the 1099-R from the payer and your return must match.

Enter a 1099-R here:

Federal Taxes,
Wages & Income
(I'll choose what I work on - if that screen comes up)
Retirement Plans & Social Security,
IRA, 401(k), Pension Plan Withdrawals (1099-R).

OR  Use the "Tools" menu (if online version left side) and then "Search Topics" for "1099-R" which will take you to the same place.

Be sure to choose which spouse the 1099-R is for if this is a joint tax return.
Be sure to pick the correct 1099-R type: Standard 1099-R, CSA-1099-R, CSF-1099-R, RRB-1099-R.

[NOTE: When you get to the "Your 1099-R Entries" screen where you can add another 1099-R, use "continue" to keep going as there are additional interview questions after that screen in most cases. You can always return as shown above.]

Call the 1099-R issuer and ask what happened.

I have contacted them and they are trying to find out what happened to the money, since the check was issued in Feb 2018.  They have no guarantee that I can get a duplicate check.  They also could not guarantee the time frame.  I asked them about the tax question and they told me that I have to show it as income whether I receive the money or not.  Now, what should I do?

So when I get to the screen where I list the standard 1099-R form, the next page gives me bank options.  What is that and how do I respond?

Bottom option:  "I'll enter it myself".
