Your advisor is incorrect. A 401(k) distribution, rollover or not, goes on line 12a/b. Only IRA's go on line 11 and a 401(k) is not an IRA.
A 401(k) to Roth is also a conversion and should be fully taxable. The 1099-R box 2a should have the same amount as box 1 and a code G in box 7.
