If my Roth IRA contributions are listed on my W2 in Box 14 (NOT Box 12), do I list them separately anywhere on my tax return? It is not an employer-sponsored account.

Yes, if you made Roth IRA contributions through your employer, essentially by direct deposit of a portion of your paycheck to your personal Roth IRA, enter them under Deductions & Credits -> Retirement and Investments -> Traditional and Roth IRA Contributions as regular personal Roth IRA contributions.  It is no different than receiving the money as part of your paycheck and depositing the money into the Roth IRA yourself.  The direct deposit is merely a convenience offered by your employer.
