AZ form 348 is used to report a credit for contributions to certified school tuition organizations.
To take this credit, your must first have maxed out your credit for contribution to private school tuition organizations (AZ form 323), then the excess credit allowed will be reported on form 348 automatically.
In TurboTax, you must indicate that you made a contribution to an Arizona private school:

Thank you for the answer. I am disappointed that it is no longer available in the online version, as I have used it in the past. At least now I know I didn't miss it.
In the online version of turbo tax I am not being directed to where I can put the balance of the allowed $1219 individual single max credit allowed. Turbo Tax allows " I donated to an Arizona private school by April 18, 2022" for $611 and not the balance of $608 . The is no reference to AZ Form 348
Enter your total contributions in the box @joellohr. Unlike the IRA section, "by April 18, 2022" does not mean January 1 to April 18. It means all your contributions.
Response was not relevant on how to get to form 348 in turobtax. I does not solicit a query response when searched. Turbotax automatically bring up 323 to max out, but then does not direct user to 348 for inclusion of the remainder controibution, in my case only $611 of $1219.
If you read Thomas' answer above "To take this credit, you must first have maxed out your credit for contribution to private school tuition organizations (AZ form 323), then the excess credit allowed will be reported on form 348 automatically", you will see that there is no direct path to form 348.
Instead, assuming that you are filing Single, when you enter 1,219 for "I donated to an Arizona private school..." under School Credits (see image above), you will likely see a screen in the Arizona interview that states: "Maximum Credit...Your maximum available credit for contributions to private school tuition organization (Form 323) made during 2021 is limited to $611..."
This screen goes on to state that any excess contribution will be put on form 348 and "There is nothing more you need to do."
So, to repeat, you do not go to form 348 to enter any contribution for a credit, instead, you indicate your total contributions to form 323, and if you contribute more than the max, the rest will be placed on form 348 automatically.

Nope did not work. Would be happy to share copies of the TurboTax generated form 301, 323. There is no
form 348 generated. Refund is shorted by $609.
Please send us a sanitized copy of your return (our process redacts any personally identifiable information on the return). You will do this by sending us a "token" (a number that points to your redacted tax return in our database).

TurboTax Online:
Click Tax Tools in the menu to the left.
Click Tools, and then
Click Share my file with Agent.
A pop-up message will appear. Click OK to send the sanitized diagnostic copy to us.
Provide the token number that is generated onto this thread.
TurboTax Desktop:
Select Online menu if the customer is using Windows. Select “Help” if using a Mac.
Select Send Tax File to Agent.
A pop-up message will appear, and the customer will select, Send. If using Mac "Send Tax File to TurboTax Agent"
Note: Desktop will save a file to your computer unless you uncheck the box.
Another message will appear. Provide the token number that is generated onto this thread.

I want to know if they fixed this glitch. I want my credit and this program is not allowing us to get the full amount. Something shady is going on.
The first thing I noted about your return is that you are filing married separate (MFS) whereas my answer was for married filing joint or single (MFJ or S).
TurboTax is not handling MFS the same way it is handling S, so let me give you a workaround. Like most workarounds, it has its issues, but my goal is to get the right numbers on the Arizona 140.
1. Go back into the Arizona interview in TurboTax.
2. Navigate to the screen that shows "Tuition Contributions Summary"
3. As I see it, there are three entries in this table, one with 1,219 and the other to with zero contributions.
4. Delete the two entries with zero contributions.
5. Edit the other entry (with the 1,219).
6. Change the 1,219 to 2,442.
7. At the bottom of the Summary screen, click Done.
8. The next screen should be "Maximum Credit". This will tell you that there is $611 on your form 323 and $607 on your Form 348.
I tried to get to the max of $608 for form 348, but because of the arithmetic, I can't get it any higher than $607.
At least I got you within $1 of your actual credit. If you have the CD/download software, feel free to play with this. If you find a way to get the $608, let us know.
Some of the entries on your forms may seem a bit odd, so you may (I stress may) get a letter from the AZ DOR asking about this. Just reply the truth, that you had to enter it this way to get the software to do what you wanted.

Thank you for the quick and detailed reply. It is unfortunate that the Turbo Tax software is not robust enough to ignore zeros and requires a false entry that does not reflect my actual donation (2442false vs 1219actual). I will submit the tax as is, and seek the help of an accountant next year to recover the balance of the AZ credit earned. To suggest that human review of my content finds issues that the robotic reviews do not indicates sub-optimal software development.
This year's experience, after ten years of trouble-free Turbo Tax use, is most troubling. I do not expect to continue use of this Intuit product, and would not be able to suggest it to any others. Intuit has either failed to keep up with complexity of State tax content or has not maintained the highest level of programming and coding talent during the pandemic.
