If you read Thomas' answer above "To take this credit, you must first have maxed out your credit for contribution to private school tuition organizations (AZ form 323), then the excess credit allowed will be reported on form 348 automatically", you will see that there is no direct path to form 348.
Instead, assuming that you are filing Single, when you enter 1,219 for "I donated to an Arizona private school..." under School Credits (see image above), you will likely see a screen in the Arizona interview that states: "Maximum Credit...Your maximum available credit for contributions to private school tuition organization (Form 323) made during 2021 is limited to $611..."
This screen goes on to state that any excess contribution will be put on form 348 and "There is nothing more you need to do."
So, to repeat, you do not go to form 348 to enter any contribution for a credit, instead, you indicate your total contributions to form 323, and if you contribute more than the max, the rest will be placed on form 348 automatically.
