That's a 1099-INT. Doesn't it say 1099-INT at the top, or under the OMB number?
To enter it in TurboTax, type "1099-int" (without the quotes) in the Search box, then click the link that says "Jump to 1099-int."
According to the OMB, this is a statement of how much interest you were paid. You need to report this interest as if it were on a 1099-INT.
