What is a 10-95a form and where do I get it at??

You only get a 1095-A if you bought health ins from the Marketplace, Obamacare. If you got  a 1095-B or 1095-C you don't enter it on your tax return.  If you didn't get a 1095A then say no and skip the heath care question.
