What if you do not know the basis of retirement income for PA state?

Short answer, you need to ask your plan administrator, he may be able to help.

Otherwise, I'd recommend you file for an extension until you can properly complete your tax return.

For PA tax purposes, your basis in a retirement plan is the sum of all your contributions over the years less the sum of any distributions taken in previous years.
