That's not really a backdoor Roth and the transfer into a separate IRA first is an extra unnecessary step that has no effect. Your conversion of 16K into a Roth will not be tax-free.

Yes, my husband did this incorrectly without consulting me and by working with an "advisor" who received fees on the unnecessary steps. This is now causing tax preparation headaches and extra taxes that could have been avoided. Anyway, we have always known this will be taxable but I want to ensure I enter correctly. I am fine entering the 1099-R but confused about other entries required.

if you moved 16k by trustee to trustee transfer or within the same custodian , then enter the 1099-R for Roth conversion and that is all.
