We never received a 2210Al form and we receive our income monthly from 1/01/2019 to 12/31/2019. What are we to do with this form?

The form 2210AI  is nothing you "receive" or get ... it is part of the income tax return to be used ONLY IF  you owe more than $1000  AND  your income is received during the year in uneven amounts.  You are not required to use it ... so not sure why you are asking about it.
