Turbo Tax has referenced wrong or different box numbers than whats on my csa 1099r. Can anyone help?

Box numbers on my csa 1099r only go up to 13. TT is asking for numbers from boxes 14, 15 and 16

Please make sure you selected the correct Form on the "Who gave you a 1099-R?" screen:

1. Login to your TurboTax Account 
2. Click on the Search box on the top and type “1099-R”
3. Click on “Jump to 1099-R” 
4. On the "Who gave you a 1099-R?" screen select "Office of Personnel Management (CSA 1099-R) For U.S. Government Civil Service retiree annuity"

Yes, I did check (click on) that box: "Office of Personnel Management (CSA 1099-R) For US Government Civil Service retiree annuity" and the pages that followed had all the same (correct) boxes that I have on my 1099-R when I filled in the information.

The issue I'm having is happening during TT's review of my income information. It flags this one item and the boxes it refers to on this page do not exist on the 1099-R. The title at the top of the page says, "Let's Verify This Entry." These boxes are not on my 1099-R

It states that this is information got carried over from the prior year tax return. Please check if you have another 1099-R listed on your return:

1. Login to your TurboTax Account 
2. Click on the Search box on the top and type “1099-R”
3. Click on “Jump to 1099-R” 
4. Delete the 1099-R that got carried over.
