The IRS/SEP/SIMPLE box on my 1099-R form is checked Y. What does this mean?
It means it is not from a 401K plan. So it will show up on 1040 line 15a and 15b and not on line 16. Make sure you check that little box on the screen when you enter the 1099R.

My 1099-R does not have any check box for the IRS/SEP/SIMPLE box on my 1099-R

Then if you don’t have that box you do not check it. Why did you get a 1099R? From what plan?

My husband had the survivor benefit plan on his retirement which activated upon his death.

That is not an IRA so you don’t check the IRA box (which you don’t have anyway).

There is not a box to mark (IRS/SEP/SIMPLE). It only states 28018 form 1099-R and the box next to it states: Distributions From Pensions, Annuities Retirement, or Profit-Sharing Plans, IRS, Insurance Contracts, etc.

That is NOT an IRA so ignore the IRA/SEP/SIMPLE box in TurboTax - not not check it.

Not really. Line 28 is for self-employed retirement *contributions* to various types of retirement accounts, only some types of which are IRAs, while a Form 1099-R with the IRA/SEP/SIMPLE box marked indicates a *distribution* from an IRA. You could make a self-employed contribution to, say, a SEP-IRA or SIMPLE IRA and later make a distribution from that same IRA, but the contribution and distribution are separate transactions.

The IRS/SEP/SIMPLE box on my 1099-R form is checked Y. What does this mean?
"Enter an “X” in the IRA/SEP/SIMPLE checkbox if the distribution is from a traditional IRA, SEP IRA, or SIMPLE IRA. Do not check the box for a distribution from a Roth IRA or for an IRA recharacterization." (Instructions for Form 1099-R)
