I did not receive a 1099 from a withdrawal from a retirement plan, but did get a letter saying the amount would be reported to irs...how do I file this?

The firm is obligated to provide a 1099R to both you and the IRS.  Did you contact them by phone?  Do you have an on-line account where you can print your own 1099R?
