My Former Employer's Retirement Pension is sent to me by a benefit payments service. Do I list The Employer or the services the payer?

You will want to enter the name on the 1099-R, which should be the Adminsitrator (Payment Service)- it must match the EIN number that the IRS has.

Thanks! I discovered that after asking as the only address and tax ID was for the Benefits Service company. Your answer was spot on!
