My ex-wife passed away and left her part of my pension to my 12 yr old grandson. Does he have to pay taxes on this?

Inherited retirement plan funds have specific rules and the plan administrator should be advising you on what can and cannot be done ... depending on the amount in the account the path to take could be different especially at the age of the child.
